Draw a Circle into a Bitmap

I need to draw circle into a bitmap and save or the bitmap or the position of this circle, but I not Know How make it
I explain :

when the doctors create a new record must sign the part of the body of man/woman where the patient is affected by pathologies



It is too hard to make because when I insert a bitmap and move a bitmap erase the background into where thereis not a same background (see th epicture lines black)

How Insert text ( as you can see in picture)

any Idea or test sample ?

why ROUND ???
It is possible, to capture only the defined Area or move a Frame inside the Image and save the Image
with the frame on a selected Posion and Size. The Frame can have just a small Border or You keep the Title and
include a Info.

I must insert, erase an bitmap into this image and save the final result ,it is link to a record
