Need to develop an information gathering web page

I need the data saved from the page in a comma delimited file to be automatically mailed to me.

Any thoughts on how to do this or if there are any programs available on doing it.

Help would be greatly appreciated.

If your web server has PHP support, then you can easily do it with some PHP source lines.

What web server are you using ?

The site is PHP capable. Also our site is hosted by Vodahost and our domain name is gohagit.com.
